Description of activity

This is specialist service which provides support and care management for adults with an Autism Spectrum Condition (ASC). This is also known as Autism Spectrum Disorder (ASD).

The service also supports adults with mental health difficulties and a dual diagnosis of mental health needs and ASC or ASD.

The service helps adults through 1:1 support or signposting to other services.

Assessments are undertaken and, depending on need and circumstances, the Team may be able to provide the following services:

Employment
Housing advice
Drug and alcohol support
Debt management
Budgeting
Correspondence
Psychiatry and psychological support
Family and Carer support and Carers assessments are also available.
For adults with ASC

All adults with ASC are entitled to an assessment of their needs
Early intervention and transition planning for young people with a diagnosis of ASC known to children's services as they move into adulthood
Care management for adults with ASC who meet the threshold for adult social care
All adults with a diagnosis of ASC that are eligible for adult social care will be entitled to a personal budget including the option of direct payments. This enables individually tailored support
All adults without a clear diagnosis but believed to be on the spectrum will be entitled to a sign posting service
Employment support is provided
Independent advocacy provision can be provided
All adults, including those with ASC regardless of the eligibility threshold, are entitled to information and advice
Support group provision is available in a variety of settings

The service is for adults 18+ with an Autism Spectrum Condition (ASC). This is also known as Autism Spectrum Disorder (ASD).

The service also supports adults with mental health difficulties and a dual diagnosis of mental health needs and ASC or ASD.
